2. One of the main reasons for dry eye symptoms and Meibomian gland dysfunction is a build up of microbial flora on the eye lashes and inside the Meibomian glands. This condition is called blepharitis. If lids are quite inflamed ,red and crusty then proper lid hygiene is crucial as the first step at natural dry eye relief. I recommend scrubbing the lids and lid margins with a commercial preparation like occusoft lid scrub foam. This scrubbing will eliminate much of the bacterial flora on the lashes that can clog the Meibomian glands that secrete the crucial outer layer of the tear film. Often times if a patient has a chronic irritated red-eye even after doing lid scrubs with occusoft they may have another condition causing the problem, Another type of blepharitis is related to a small parasite called demodex. Demodex is a small parasite ( mite)that crawls into the hair follicle at night and lays eggs. These eggs will back up on the base of the lash causing a collarette to be seen by the optometrist or ophthalmologist with the bio microscope.  The demodex may cause lids to become red, inflamed, crusty , and itch. It is important to wipe the lashes and lid margins with a special pre medicated pad called cliradex right before bed. Cliradex has the demodex killing properties derived from tea tree oil and is extremely effective for the treatment of demodex related blepharitis and secondary dry eyes. Scrub lashes first with occusoft, rinse then rub the Cliradex onto lashes and lids before bed and do not rinse. This combination of lid hygiene will help to eliminate the microbial flora ( staph and demodex) that is blocking the Meibomian glands and causing the dry eyes. 3. Meibomian glands often times need a jump-start to help them function to their fullest potential and secrete a healthy tear film. Massaging and heating the lid margins is necessary to unclog the Meibomian glands so these glands can secrete the outer lipid layer of tear film. This needs to be done for 10-15 minutes 2 times a day. The use of a warm wash cloth is an inexpensive way to accomplish this but Dr. Lange recommends using  the Bruder Mask to heat up the Meibomian glands.  This is a mask that is put in microwave for 20 seconds and then Velcro around your face for 10-15 minutes two times a day. While the mask is on the eyes gently massage the lid margins to help un-block the blocked glands. This mask will also help cut inflammation surrounding the eyes. The Bruder eye hydrating mask can last 5-10 years without being replaced. Artificial tears that have some hyaluranic acid for dry eye relief is Dr Langes first choice. 5.  If these first four steps are followed a large percent of dry eye /Meibomian gland dysfunction patients will improve and become asymptomatic. However there will still be patients  that have resistance and will still suffer from this condition. This is when punctal occlusion is recommended.  This is done by your optometrist or ophthalmologist in the office in less than five minutes.  The doctor will take a small custom sized soft medical silicone implant and place it in your drainage duct in your lower eye lids and sometimes in the ducts in the superior eye lid . This is a painless quick procedure that is covered by your insurance.  This blocks the duct that the tears drain out of. This procedure keeps the natural tears on the eye much longer bathing the eye in the natural lubricating properties of the human tear. Punctal occlusion therapy is much more effective after patients have been on anti inflammatory omega 3 fish oil, astaxanthin and GLA for a period of two to three months.  The anti inflammatory nutrients reduce the inflammatory markers in the tear film which makes punctal occlusion much more effective. 6.  Even after following the first five steps you will have patients with moderate to severe dry eye that still have superficial punctate keratitis and significant symptoms.  These patients will usually respond extremely well to new generation stem cell therapies. Blepharitis is a chronic inflammatory condition of the eye lid margins.  Dr Michael Lange says this condition can be very mild and symptomless or marked with severe symptoms.  Belpharitis is a combination of scaley, flakey, red , swollen, crusty,  greasy eye lids.  Patients with blepharitis are more likely to experience eyes that burn, tear, feel sandy and gritty these patents often times wake up in the am with their eyes matted together. Blepharitis also increases the risk of a sty or chalazion forming on the lid margins. Many of these patients will also loose lashes along the lid margins.

There are three basic types of blepharitis. 1. Bacterail,  2. Seborrheic   3. Demodex ( Mites).

Natural treatment for blepharitis consists of proper lid hygiene. Scrubbing the eye lashes extremely well before bed time with occusoft foam will reduce the cause in most cases however a certain percent of patients will have demodex ( mites) on their lashes and will need to use a diluted solution of tea tree oil applied to lid margins or will need to use Cliradex a commercially prepared wipe with the components  of tea tree oil in it already.  The eye lid margins must first be heated well with a warm wash cloth or use the Bruder mask ( mask developed to heat the lids) , then clean lashes well with occusoft scrubbing foam and rinse well. Prior to bed wipe the cleaned lids with Cliradex wipes and do not wash off. The Cliradex will kill the demodex mites that are laying eggs in the hair follicles and creating a marked case of blepharitis..  So heating and cleaning the lid margins is the most important step at treating blepharitits.  When the heat is being applied to the lid margins gently massage the lid margins to help open the potentially blocked Meibomian glands along the lid margins.

Dr Michael Lange uses Fortifeye super omega two times a day orally in his patients to help reduce the inflammation associated with blepharitis. He states this is a crucial step in the natural treatment of blepharitis. Most of the blepharitis patients will have a dry eye as well so using a good unpreserved artificial tear if very important. Most of the artificial tears purchased over the counter do not work well however. Dr Lange recommends using a preservative free lubricating eye drop that has hyluranic acid in it like Oasis Tears Plus or Blink tears. These both work quite well in dry eye patients with blepharitits. In the more advanced dry eye patients he recommends using one of those two (blink or Oasis) combined with Retaine tears. Retaine replaces the outer lipid layer of the tear film that may be compromised in blepharitis patients.

In summary the natural treatment of  Blepharitis is Heating the lids, cleaning the lids and utilizing the nutritional benefits of Omega 3 fish oil and astaxanthin. Michael Lange OD on omega 3 and prostate cancer risk.

This is simple, no correlation at all. Good studies prove that people that consume more omega 3 are less likely to have prostate cancer !  If this ridiculous theory was correct then the groups of people that consume a lot of fish would have super high levels of prostate cancer, but this is just the opposite they have less prostate cancer. Dr. Michael Lange optometrist with Lange Eye Care discusses  Dry Eye Nutrition.  We are all plagued with dry eye symptomatology at some time in our life.  Eyes that feel sandy, gritty, burn , tear, itch, fluctuation in vision and recurrent infections can all be related to dry eyes.  Medications, age, poor diet, certain systemic diseases, ocular surgeries, contact lenses , particular job , hobbies and sports can all compound the problem of dry eyes.  First and foremost the typical American diet needs to be overhauled.  Most Americans are eating a diet that is caloric dense and nutrient depleted.  Most Americans are eating a pro inflammatory omega six diet that creates inflammation in the body that can lead to dry eye syndrome.  The quickest treatment for dry eyes is to use a good unpreserved artificial tear like oasis tears plus three to six times a day combined with good lid hygiene.  Warm compress to the eye for ten minutes three times a day while massaging the lid margins will help to break down the clogged Meibomian glands that will create a dry eye.  Lid scrubs once a day with occusoft lid scrubs before bed will also help to eliminate the bacterial and viral flora that clogs the Meibomian glands.
